Is chocolate bad for your teeth?

The sugar is the problem, as it is with any sugary food. Claims that cocoa components protect teeth are a long way from established.

What the evidence shows

Dietary free sugars are established by dental and public health bodies as the principal dietary cause of tooth decay, and chocolate is a sugar-containing food. Separately, laboratory work has reported that certain cocoa constituents — including theobromine and some polyphenols — show activity against oral bacteria or against enamel demineralisation in vitro, and a small number of trials have examined theobromine-containing dentifrices.

What it does not show

It does not show that eating chocolate protects teeth. Laboratory activity against oral bacteria does not survive the presence of the sugar delivered alongside it in a real product, and no trial establishes a net protective effect from eating chocolate. Claims that chocolate is 'better for teeth than other sweets' because it clears the mouth quickly are plausible in mechanism and not established in outcome.

How good is the evidence

The sugar-and-decay relationship rests on systematic reviews and public health guidance and is not in dispute. The cocoa-protective side rests largely on in vitro work and a small number of dentifrice trials, several with commercial involvement, and is a long way from supporting a claim about eating chocolate.

Who this may not apply to

  • Dental advice is clinical and individual. Nothing here substitutes for a dentist.
  • Children are the population the comparative question is usually asked about, and the comparison between confectionery types is less well studied than the underlying sugar relationship.

Is chocolate better or worse than other sweets?

This is the question people actually arrive with, and the honest answer has two halves.

The mechanism half is genuinely plausible. What drives decay is not the quantity of sugar alone but how long teeth are exposed to it, and confectionery differs sharply in that respect. Chocolate melts at body temperature and clears the mouth relatively quickly. Hard-boiled sweets are sucked over long periods, and toffee and dried-fruit sweets adhere to tooth surfaces. On exposure time, chocolate compares favourably.

The evidence half is weaker. Studies comparing decay outcomes between confectionery types in real diets are scarce, hard to control and hard to separate from everything else a person eats. So the mechanism supports the comparison and no trial establishes it.

What that adds up to is a plausible mechanism rather than a finding, which is exactly what this topic's tier says. It is not a reason to describe chocolate as good for teeth, and it is not nothing either.
