Why ganache splits, and how to bring it back
A broken emulsion, not a ruined one. Almost every split ganache can be rebuilt, and the method is counterintuitive.

A split ganache is a broken emulsion and is nearly always recoverable: add a spoonful of warm liquid and stir from the centre outward, or use a stick blender.
What a split is
A ganache is an emulsion: fat droplets dispersed through a water phase, stabilised by the cocoa solids and by the milk proteins in the cream. A split means that dispersion has collapsed and the fat has coalesced into a separate layer.
The usual triggers are temperature shock — very hot cream onto cold chocolate, or the reverse — over-stirring while hot, or a ratio with too much fat for the water phase to carry.
The rescue
The instinct is to add more chocolate or to beat harder. Both make it worse.
What works is adding a small amount of warm liquid — a tablespoon of warm cream or milk — and stirring from the centre outward in small circles. Starting at the centre rebuilds a small stable emulsion and gradually incorporates the rest, where stirring the whole bowl just moves the separated fat around.
A stick blender is the reliable version of the same thing, and it recovers ganaches that hand stirring will not. Keep the head submerged to avoid whipping air in.
Prevention
Match temperatures. Pour hot cream over chopped chocolate and let it stand for a minute before stirring, so the chocolate melts in the cream's heat rather than being shocked by it.
Stir gently and from the centre. And check the ratio against the chocolate type — a split is sometimes really a ratio problem showing itself as an emulsion failure, particularly with white chocolate, which carries the most fat and the least stabilising solid.
